First time users: Please complete the Portal Account Request Form and fax, mail or bring to the office. We will activate the account for you with the completed documentation.
Requests may take up to five business days to complete. Once complete, you will receive an email from “firstname.lastname@example.org” with a link to the portal as well as a temporary password which is valid for one week. Please note: Gmail will automatically place the email in your spam folder.
Once set up you can check future appointment date/time, date of last physical, immunizations, labs and request medication refills!
